Under the direction of the Director of Nursing, the Social Service Designee is responsible for the coordination and direction of the social service program of the Skilled Nursing Facility (SNF). Identifies the medically related social and emotional needs of the resident and arranges for the meeting of these needs through individual and group social service programming and appropriate referrals to outside social service agencies, community resources and professional services.

Compensation for Benefited, Full-TIme Pay Scale without a CNA
- Base Rate: $26.68–$38.04 per hour (or annually $55,494.40 - $79,121.74)
- Starting pay is determined using our clinical pay scales, which feature 13 steps recognizing up to 30 years of relevant experience.
- Eligible employees may also receive shift differential and holiday pay.
Compensation for Benefited, Full-TIme Pay Scale with a CNA
- Base Rate: $28.26–$40.29 per hour (or annually $58,780.80 - $83,807.37)
- Starting pay is determined using our clinical pay scales, which feature 13 steps recognizing up to 30 years of relevant experience.
- Eligible employees may also receive shift differential and holiday pay.
Required Education:
- Social Service Coordinator Certificate within three (3) months of hire.
- Obtain CA Certified Nursing Assistant license within six (6) months of hire
Preferred Education:
- Current CPR Certification

- Is able to communicate in English both verbally and in writing.
