Plumas District Hospital strives to provide you and your family with a comprehensive and valuable benefits package. We want to make sure you’re getting the most out of our benefits.

Benefits are offered to Full-time employees working at least 72+ hours per 2 week pay period and Part-time employees working 36+ hours per 2 week pay period. Family members who are eligible for coverage are Legal spouses, Registered domestic partners, and Dependent children under age 26.

Most of the benefits listed below must be accepted or declined upon eligibility after the waiting period. The decision to participate may not be changed until the Annual Enrollment Period at the end of the calendar year, unless you experience a Qualifying Event.

Paid time off (PTO) program

Full time employees accrue Paid Time Off (PTO) hours each year. This PTO is used for holidays, vacations, family emergencies, etc., at the employees’ discretion and with the permission of their supervisor. Part-time employees accrue at a pro-rated amount, per hour of service. Employees begin accruing PTO immediately upon employment in a benefited status, and may start using accrued hours following a 30-day waiting period (exception for use during PDH recognized holidays).

Accrual schedule

  • <4.999 yrs. of service=200 hours/year
  • 5-9.999 years = 240 hours/year
  • 10 or more years = 280 hours/year

Paid Sick Leave program

All employees who are anticipated to complete 30 days of employment during the calendar year receive Paid Sick Leave hours beginning with their 30th day of employment or what equates to 30 days for per diem employees. Five days are frontloaded based upon the anticipated work schedule. 
Bereavement / Jury Duty Leaves (paid to benefit-eligible with certain limits and eligibility).

Health Benefits

Available after 30 days of continuous service in a benefited status. We offer a comprehensive Health Benefits package (Medical & Pharmacy Plan, Dental Plan and Vision Plan).Employees contribute to a portion of the cost, with the Hospital picking up the majority of the cost. Employee contributions are made via bi-weekly payroll deductions on a pre-tax basis. Employees can cover themselves and all eligible family members. No more bundles.

The PDH PPO and pharmAcy plan

$2,000 deductible ($4,000 Family).90% coverage at PDH, 70% at a PPO Network Provider, and 30% at a Non-Network PPO Provider.$20/$35/$50/20% Pharmacy program.

Metlife Delta Dental pDH Plus

$50 deductible, 100% for Exams & Cleanings, 80% for standard fillings & extractions, and 50% for crowns, bridges, etc., at a participating MetLife provider. $2,000 Maximum Benefit payment annually. Orthodontics (Adult and Child) 50% coinsurance with a $2,000 lifetime maximum per patient.

Metlife Vision Choice

$20 copay for an eye health exam and $60 copay for a Contact exam every 12 months. Frame allowance of $140 every 24 months or elective Contact allowance of $140 every 12 months. Frame allowance is $75 when purchased at Costco, Walmart or Sam's Club.

Basic Life Insurance

Available at the first of the month following one month of continuous service in a benefited status. $20,000 Basic Term Life and matching AD&D insurance (coverage is Employee Only).

Voluntary Life Insurance

Depending on your needs, you may want to consider buying supplemental coverage. With voluntary life insurance, you are responsible for paying the full cost of coverage through biweekly payroll deductions. You can purchase coverage for yourself or for your spouse; the minimum coverage level is $20,000 and the maximum is $500,000 or 5 times your annual salary (whichever is lower). Your spouse is eligible for 50% of your life insurance amount. Children are eligible for up to $10,000.

Voluntary Long Term Disability

Benefitted Full-time employees can purchase Voluntary Long-term Disability insurance. Long-term disability is intended to protect your income for a long duration after you have depleted any state provided short-term disability or any sick leave.

Flexible Spending Accounts (FSA)

Available after 30 days of continuous service in a benefit-eligible status. These employees are eligible to participate in the Flexible Spending Account program for healthcare expenses and/or qualified daycare reimbursement. Through payroll deduction on a pre-tax basis, employees may utilize this plan to set aside money for out-of-pocket healthcare expenses, dependent care provider costs, or both.

Retirement Plans

Employees may start, stop and change contributions at any time. Plumas District Hospital will contribute 3% of gross wages to a retirement plan for employees beginning the first full pay period after working for one year in a benefit-eligible status.

Employee Discount on Bills

After 30 days of continuous service in a benefitted status, employees and their spouses, registered domestic partners, and/or dependents will have any deductibles and coinsurance incurred for services received at Plumas District Hospital waived. You must also have qualified insurance either through PDH or another plan, be obtaining a service which is otherwise covered by your insurance and which is medically necessary.

Continuing Education Benefits

Tuition Reimbursement Program: Qualified employees, after one year with us, can receive up to $400/semester reimbursement based on passing grades and proof of expenses incurred.

LVN to RN Step Up Program: To further your nursing career, Plumas District Hospital will support qualified LVN's by accommodating changes your employment status, guaranteeing a full-time RN position after completion of courses and receiving licensure, and paying a total of $5,400 dollars for earning your nursing degree. 
Plumas District Hospital may financially support other relevant further education requests. 

Continuing Education Days (After one year with us as a Full time or Part time benefited employee): CE credits are provided through the form of five full paid days every two years for full-time staff and two days every two years for part-time staff. Allied Health Providers meeting certain criteria receive annual reimbursement for CE courses. We also provide CPR,ACLS, and PALS renewal courses for our employees.

  • Plumas District Hospital is eligible for the Public Service Loan Forgiveness (PSLF) program as a local government entity (due to being a district hospital). Eligibility criteria may vary across different programs. We encourage our candidates and current staff to explore the available options online.
