The Advanced Practice Provider (APP) may be a Nurse Practitioner or Physician Assistant. The advanced practice provider is responsible for the assessment, medical case management, diagnosis, treatment, follow-up and referral of clinic patients seen at the North Fork Office, Quincy Office or Indian Valley Office. The APP plans patient care using skills specific to his/her individual training and experience and works under established protocols.

Required Education:
- Must be a graduate of a nurse practitioner program that meets the standards of the Board of Registered Nursing (BRN) in California or a graduate from a physician assistant program that meets the standards of the Physician Assistant Committee of the Medical Board of California.
- Must be licensed in California.
- Current CPR certification.
Required Experience:
- Graduation from a nurse practitioner program which meets the standards of the California BRN or physician assistant program that meets the standards of the Physician Assistant Committee of the medical Board of California and completion of training consistent with the procedures to be performed.
Knowledge, Skills, and Abilities:
- Excellent interpersonal skills with patients required.
- Ability to communicate effectively with team members, patients and community members.
- Must demonstrate responsibility for keeping credentials current and for updating his/her knowledge.
- Must demonstrate reliable and punctual attendance.
- Must have a friendly and helpful disposition toward visitors and staff.

Child Care:
- PDH is thrilled to provide our Child Care Center services to all families in our area. Our program is operational from 7:45am to 5:15pm, Monday through Friday, and is approved for children aged 0 to 5 years. After submitting your application, our Child Care Director will get in touch with you about your acceptance into the program. Once approved, we will start the enrollment process.
