Benefits are offered to regular part-time and full-time employees who work at least 36 hours per two-week pay period at Plumas District Hospital.
Most of the benefits listed below, such as the Health Benefit plan, must be accepted or declined upon eligibility after the waiting period. The decision to participate may not be changed until the Annual Enrollment Period in December, unless you experience a Qualifying Event.
Paid Time Off (PTO) Program
Full time employees accrue Paid Time Off (PTO) hours each year. This PTO is used for holidays, vacations, illness, family emergencies, etc., at the employees’ discretion and with the permission of their supervisor. Part-time employees accrue at a pro-rated amount, per hour of service. Employees begin accruing PTO immediately upon employment in a benefited status, and may start using accrued hours following a 90-day waiting period (exception for use during holidays).
Under 5 yrs. of service = 200 hours/year
5-9 years = 240 hours/year
10 or more years = 280 hours/year
Wellness InitiativeAll employees are eligible for a incentive-based wellness rewards program. Eligibility is the first of the month following 30 days of employment.
Available after 30 days of continuous service in a benefited status. We offer a comprehensive Health Benefits package (Medical & Pharmacy Plan, Dental Plan and Vision Plan). Employees contribute to a portion of the cost, with the Hospital picking up the majority of the cost. Employee contributions are made via bi-weekly payroll deductions on a pre-tax basis. Employees can cover themselves and all eligible family members. Certain benefits may be unbundled.
The PDH PPO and Pharmacy Plan
$2,000 deductible ($4,000 Family). 100% Preventive Care Benefit. 90% coverage at PDH, 70% at a PPO Network Provider, and 30% at a Non-Network PPO Provider. $20/$35/$50/20% Pharmacy program.
Delta Dental Premiere
Available if employee also selects the PPO plan coverage. $50 deductible, 100% for Exams & Cleanings, 80% for standard fillings & extractions, and 50% for crowns, bridges, etc., at a participating Delta provider. $1,500 Maximum Benefit payment annually.
Vision Service Plan (VSP)
$20 co-pay for a vision exam every 12 months. After a $20 copay, lenses covered by VSP every 12 months with a $20 co-pay; and frame costs covered every 24 months to maximum of $140.
Life and Accidental Death and Dismemberment (AD&D)
Available at the first of the month following one month of continuous service. $20,000 Basic Term Life and matching AD&D insurance (coverage is Employee Only). Supplemental Life and AD&D for Spouse and Children is also available for purchase.
Flexible Benefit Program (FSA)
Available after 30 days of continuous service in a benefit-eligible status. These employees are eligible to participate in the Flexible Spending Account program for healthcare expenses or qualified daycare reimbursement. Through payroll deduction on a pre-tax basis, employees may utilize this plan to set aside money for out-of-pocket healthcare expenses, dependent care provider costs, or both.
After one year of service at the hospital, benefited employees receive a contribution of 3% of gross wages from the Hospital to the 403(b) pension plan through the Principal Financial Group, with Morgan Stanley as the financial advisors.
Any employee (benefitted or not) may also voluntarily participate in contributing to their 403(b) retirement plan, and/or a 457(b) plan, and/or a Roth 403(b) plan. This benefit is available immediately upon employment. Employees may contribute any percent or flat amount of their gross pay every pay period, subject to annual maximums. The employee controls the investment funds on employer and employee contributions and the vesting period is one calendar year. Additional financial services are offered by the Principal Financial Group.
Benefitted employees are eligible for membership in the Sierra Central Credit Union.
Other insurance plans are available for benefitted employees to purchase at the first of the month following one month of continuous service. These include Universal Life Insurance/Long Term Care, and Long-Term Disability insurance (for full-time employees only).
Available after 30 days of continuous service in a benefit-eligible status. Employees who meet certain criteria will have their billed amounts waived, except for co-pays.
