You can submit a Confidentiality Request to inform our organization of information in your health record which you would like to be treated as confidential. Someone from our privacy team may contact you to review your request. If you are concerned about who may have access to your online health information, please contact a member of your care team. Please follow the instructions in the document below to submit a Confidentiality Request through the patient portal.
After enrollment, access your patient portal using your mobile device by downloading the HealtheLife app from Google Play or the iOS App Store.
