Skilled, attentive, home like care.

The facility provides long-term custodial care and, when fully operational, will host 36 beds and create 79 new job opportunities for our community. Frequently Asked Questions

Services at the PDH Skilled Nursing Facility are mainly covered through Medi-Cal Long Term Care benefits (Under the Nursing Home Medicaid Program). Please note that if you are eligible for these benefits there will still be a share of cost, determined by Medi-Cal. To be eligible for Medi-Cal a person has to meet certain financial requirements and functional (Medi-Cal) requirements. The financial requirements vary by the applicant’s marital status, if their spouse is also applying for Medicaid and what program they are applying for, such as the Nursing Home Medicaid Program. The easiest way to find the most current Medi-Cal eligibility criteria for your specific situation is to use the Medicaid Eligibility Requirements Finder tool.

Starting January 1, 2026, California's Medi-Cal asset limit is reinstated for older adults, people with disabilities, and long-term care applicants: $130,000 for one person, plus $65,000 for each additional household member (up to 10), counting assets like bank accounts, but excluding your home, one car, and certain retirement funds; this brings back rules that had been eliminated, requiring you to "spend down" excess assets to qualify.

You still might be requested to provide documents to verify your income from property/assets, with the Social Services Department in the county which you reside/or previously resided in.

If your coverage is denied, delayed, reduced, or stopped, you can contact your local Medi-Cal office to review your case. You can also ask for a State Fair Hearing to determine your eligibility if you disagree with the decision. Call the State Hearings Division toll-free at (800) 743-8525. If you ask for a hearing before your Medi-Cal ends, your coverage will continue during the process.
