Benefits Package
Benefits are offered to regular part-time and regular full-time employees who work at least 36 hours every two weeks at Plumas District Hospital. Most benefits are available the first of the month following three months of service.
Most of the benefits listed below, such as the Health Benefit plans, must be accepted or declined upon eligibility after the three-month period. The decision to participate may not be reversed until the annual open enrollment period, unless you experience a Qualifying Event.
Paid Time Off (PTO) Program
Full time employees accrue 200 hours (25 days) per year of paid time off during their first four years of service. This is to be used for holidays, vacations, illness, family emergencies, etc., at the employees’ discretion and with the permission of their supervisor. Part-time employees accrue at a pro-rated amount, per hour of service. Employees begin accruing PTO immediately upon employment, and may start using accrued hours following the standard 90-day probationary period. “Cash-out” options are available.
1-4 years of service = 200 hours/year
5-9 years = 240 hours/year
10+ years = 280 hours/year
Health Benefits
We offer a comprehensive health benefits package to benefited employees (medical & pharmacy benefit, dental, and vision). Employees contribute a portion of the cost, with the Hospital picking up the majority of the cost. Deductions are made via bi-weekly payroll deductions, on a pre-tax basis. Employees can cover themselves and any eligible family members. Certain benefits may be unbundled.
The PDH PPO and Pharmacy Plan
$1,000 deductible. 90% coverage at PDH, 70% at network PPO, 50% non-network PPO. $25 co-pay for physician appointments. $15/$30/$50 prescriptions. Health Reimbursement Account, where the Hospital contributes annual amounts to help pay your out-of-pocket medical expenses. $700 is contributed for Employee & Family, $500 for Employee + One Dependent, and $300 for a Single Employee. The amounts shown here are for a full year when enrolling on January 1st – the amount is pro-rated for those that start at other times of the year.
Delta Dental Premiere
$50 deductible, 80% coverage standard fillings/extractions and 50% crowns/bridges, at participating Delta provider. $1,500 is the maximum benefit payment annually.
Vision Service Plan (VSP)
$10 copay for annual vision exam. Portion of lenses covered by VSP every 12 months, portion of frames covered every 24 months.
Life and Accidental Death and Dismemberment (AD&D)
$20,000 basic term Life and matching AD&D insurance (coverage is Employee Only). PDH pays the cost of basic Life and AD&D.
Flexible Benefit Program
Benefited employees are eligible to participate in the Flexible Benefit Program, for healthcare expenses or qualified daycare reimbursement. Through payroll deduction on a pre-tax basis, employees may utilize this plan to set aside money for out-of-pocket healthcare expenses, dependent care provider costs, or both.
Pension Plan
Benefited employees over 21 years of age, after one year of service at the hospital, may participate in the pension plan through AIG Retirement. The employee contributes 2.5% of their gross pay every pay period, and the hospital contributes 5%. The employee controls investment funds and the vesting period is three years. Additional financial services are offered by AIG Retirement.
Voluntary 403b Tax Shelter plan
Also through AIG Retirement, any employee (benefited or not) may participate in the voluntary AIG Retirement plan via payroll deduction. This benefit is available immediately upon employment.
Credit Union
Benefited employees are eligible for membership in the Sierra Central Credit Union.
Miscellaneous Insurance
Various other insurance plans are available for benefited employees to purchase if they desire. These include universal life insurance, term life insurance and long-term disability insurance (full-time employees only).
